Collection and Use of Personal Information
Personal information is data that can be used to uniquely identify or contact a single person.
You may be asked to provide your personal information anytime you are in contact with us. We respect the right to privacy of all clients. We do not collect information that would personally identify you unless you choose to provide it. The personal information that you submit is shared only with those people in our clinic who need this information to attend to your questions, requests or wellbeing. We do not save personal information to use for other purposes, nor do we provide it to any other organization.
We have taken reasonable steps to ensure the integrity and confidentiality of personally identifiable information that you may provide. You should understand, however, that electronic transmissions via the Internet are not necessarily secure from interception, and so we cannot absolutely guarantee the security or confidentiality of such transmissions.
Except for authorized law enforcement investigations or other valid legal processes, we will not share any personally identifiable information we receive from you with any parties outside of our clinic.
Collection and Use of Non-Personal Information
We also collect non-personal information − data in a form that does not permit direct association with any specific individual. We may collect, use, transfer and disclose non-personal information for any purpose. The following are some examples of non-personal information that we collect and how we may use it:
We may collect information such as medical conditions, medical assessment, treatment, etc. This can be used so that we can better understand client needs and improve our practice and services.
If we do combine non-personal information with personal information the combined information will be treated as personal information for as long as it remains combined.
